Links in text and pics

How do you put a link in text or in pics. When I say in text, i don't just meen the address, I meen a link in a word like, cards.

for pics:

[url=url you want][img=image url][/url]

test:

no pictures

 

omg,wtf?it the pokeplushie didn't work and thats total spam.
