There are two effects. One is to attack twice that turn. The other is to place a counter instead of declaring an attacking. They combine in this fashion. You can place a counter instead of attacking, and you can then attack once (because you can normally attack twice). Or you can attack, and then place a counter (instead of the second attack), or you can place 2 counters and not attack at all.

At least, that's what I got from your effect.

Unless the effect specifically limits the number of counters you can place, or that a monster can hold, there is no limit. (Breaker only can hold 1 Spell Counter, for example).

The damage step is the battling phase after a monster has attacked and where you figure out who takes damage and how much damage is dealt. This means that the attack gain only lasts until that specific battle is done.

For if it's not mentioned, it depends. If the card just says "It gains x amount of attack." and ends there, the attack gain is permanent and does not die out.

If it says "Until the end of the Battle Phase", the gain lasts until the Battle Phase has ended and the player has decided to go to Main Phase 2 or end their turn.
