The Winner is Nine Breaker with The Hero at Zero Hour.

Your monster must have at least 2 effects
It gains each effect when your life points are below a certain amount.
Each effect becomes active at a different time

Everything else is up to you contestant

How about a minimum of two effects, but you can have as many effects as you like.

When this card is Special Summoned: It is returned to it's owner's hand during the End Phase: Any other monsters Special Summoned this turn are destroyed. During your Battle Phase: This card must attack first, and attacks all monsters your opponent controls: None of your other monsters can declare an attack. While this card is face-up on the field, apply one of the following effects:
●Life Points below 4000: When this card attacks, your opponent cannot activate any Spell or Trap Cards until after the Damage Step.

Never-Ending Nightmare Dragon

Lore: You can Special Summon this card from your graveyard by paying 500 life points. This card is not destroyed as a result of battle by a monster with 2400 or more ATK (damage calculation is still applied). If your life points are lower then your opponent's life points this cards ATK is doubled. If your life points are lower then 4000 this card is unaffected by any Spell or Trap cards that target this card.
