The time limit was agreed to by both parties as an amendment to the game rules before the duel with Pegasus began. The loss was legitimate.

 

Living Arrow wasn't cheating; the first season uses different rules, and in the manga is explicitly an RPG played with the cards rather than the regular game.

 

Resignation is a loss.

 

The duel with Joey where they were chained to the anchor was a draw, not a loss; that was why they were both able to escape. The loss to Joey was the duel begun at the end of the third season, which, while not shown on screen, could not have ended in anything but a loss for Yugi without completely negating Joey's entire character arc that had been running for the last two seasons.

It wasn't shown on-screen. We just see them approach each other and start, but it ends before any card is played.

I don't remember that either. And I don't know exactly which one is season 3 so ... :D

I don't remember what card that is, but depending on how it's worded, the other monster would gain ATK until it exceeded The Wicked Avatar's ATK by 300 - and then The Wicked Avatar's ATK would go up by 400, and it would end there.
